NSAA Pre-Participation Physicals
The first day of fall practices is approaching fast. Make sure your student athlete has their NSAA Pre-Participation Physical completed and turned in to the office prior to the first day of practice (August 7th). Physicals are required for all student athletes in grades 8-12 - forms can be downloaded using the link below. Students entering grade 9-12 will also need a Parent/Student Consent form along with their physical form (which can also be downloaded using the link below).
All students entering the 7th grade are required to have a physical exam and receive a tetanus booster.
